Sites@Duke Pro uses Qualtrics to create and embed webforms.

Create your own surveys, polls, questionnaires and forms using Duke Qualtrics.

Then, add these forms to your site content one of two ways:

1. Create a link directly to the form's URL.

2. Embed your forms within a Page, using the Iframe content block.

  • Note: Embedding a NetID-protected Qualtrics form can generate a login error and is not recommended. Instead, link directly to the form.

Other Forms Products

Sites Pro does not integrate directly with Kuali Build or other third-party form builder products. Only Qualtrics can be embedded into a Page using the iFrame block.

If using another form product, we recommend linking off to the form URL.
